Overview
Immunization programs are crucial for reducing child morbidity and mortality but face challenges like zero-dose children, incomplete vaccinations, supply chain issues, workforce constraints, financial limitations, and vaccine hesitancy, exacerbated by COVID-19. Digital health interventions offer solutions to enhance immunization services. Gavi’s Digital Health Information (DHI) Strategy promotes an equitable, gender-intentional system to identify and support zero-dose and under-immunized children. Ethiopia developed a Costed DHI Roadmap in 2022 to prioritize digital interventions. This baseline report assesses the implementation status, enablers, barriers, gender inclusiveness, and impacts on immunization outcomes to track progress over the next 18 months

Methodology/Approach
A cross-sectional study using quantitative and qualitative approaches was conducted from December 2023 to April 2024 to assess the baseline implementation status of priority DHI for immunization in Ethiopia. Data sources included desk reviews, key personnel interviews, and national survey data (e.g., Penta 1, Penta 3, MCV 1, zero dose, vaccine stock-outs, and wastage). The study explored the scope and scale of DHI interventions, enablers, and gender-related activities in Ethiopia. Indicators were derived from the Ethiopian DHI roadmap, focusing on six priority areas, enablers, and gender-related activities. Data were collected from institutions like the MoH, EPSS, EPHI, and John Snow, Inc., through document reviews, stakeholder interviews, and national surveys. DHI intervention scope and scale were analysed using roadmap-based indicators and presented in textual and tabular formats. Ethical approval was obtained from the University of Gondar IRB, with informed consent secured from participants.
